To dispose of UPS batteries, please download the Hazardous Materials Pick-up Request Form (pdf) (excel) and follow these instructions:

• Department will fill out the UPS battery form and email to: Ehso@ouhsc.edu. Please fill out the top portion of the form and list how many batteries in line 10 on the form.
• EHSO will then contact the department and schedule a time to pick up the batteries. EHSO can be reached at 271-3000.
• Moving services has been notified to not pick up any UPS batteries on campus.
